What mattress is best for back pain?
How to tell mattress-related pain from everything else, and what to change.

Support versus firmness
Support is whether your spine stays aligned. Firmness is how the surface feels. A plush mattress over a strong support core can be fully supportive, and a rock-hard one can leave your lumbar curve completely unfilled. Judging a mattress by how firm it feels in the first thirty seconds is the most common shopping mistake made by people in pain.
Match the fix to the pain
Different pain patterns point at different fixes.
- • Lower back, worst on waking — support is inadequate; try medium-firm
- • Shoulder or hip, side sleeping — the surface is too firm; more contouring or a topper
- • Neck — usually the pillow, not the mattress
- • Pain all day or radiating — see a clinician before buying anything
Check the foundation first
Before replacing a mattress for back pain, look underneath. Bowed slats, a cracked center rail or a missing center support create exactly the dip you would blame the mattress for — and cost a fraction to fix.
