Test the mattress on the floor
One night flat on the floor tells you whether the sag is the mattress or the base. It is free and it is definitive.
Center support is the usual culprit
Queen and larger beds need a center rail with a leg to the floor. Without one, the middle dips in months.
Measure impressions properly
Straightedge across the bare surface, no weight, measure the gap. Most warranties trigger at 1 to 1.5 inches.
